Do you need a referral to make an appointment? If you plan on submitting the cost of your services to your insurance company - it is best to contact your insurance company prior to see if they REQUIRE the referral. Asking them if you need a referral for Podiatry, Custom Made Orthotics, etc.

Please come 10 minutes prior to your appointment to fill out your New Patient Form.
